Request for Out of Distict Transportation

If the student is not registered in Westerly or has a change of address, the parent or guardian must contact Central Registration at 401-315-1502 for address verification.

Complete the Request for Out of District Transportation form and send it via Email or fax.

Forms need to be submitted annually by August 1 for the academic year.
